Group Fitness Class Liability Waiver

I hereby acknowledge that I have enrolled in the group fitness classes membership provided by Strive, located at 2408 Ashley River Rd Charleston, SC. In consideration for being allowed to participate in unlimited group fitness classes, I agree to the following terms and conditions:

1. Assumption of Risk: I understand and acknowledge that there are risks involved in participating in any fitness program or activity. These risks include, but are not limited to, bodily injury, illness, disability, and death. I voluntarily assume all such risks, both known and unknown, arising from my participation in the group fitness classes.

2. Health and Medical Condition: I hereby represent that I am physically fit and have no medical condition that would prevent my full participation in the group fitness classes. I agree to inform the instructor of any pre-existing medical conditions, injuries, or physical limitations that may affect my participation in the classes.

3. Release of Liability: I hereby release, waive, discharge, and covenant not to sue Strive, its owners, employees, instructors, and agents (hereinafter referred to as "Released Parties"), from any and all claims, liabilities, demands, actions, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including but not limited to bodily injury, illness, disability, or death, that may be sustained by me or any property belonging to me, while participating in the group fitness classes.

4. Indemnification: I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or costs, including attorney fees, that they may incur due to my participation in the group fitness classes.

5. Photography and Videography Release: I grant permission to Strive to take photographs and/or videos of me during the group fitness classes. I understand that these photographs and/or videos may be used for promotional purposes, including but not limited to social media, website, and marketing materials.

6. Emergency Medical Treatment: In the event of an emergency, I authorize the staff of Strive to obtain medical treatment for me, including but not limited to first aid, CPR, and transportation to a medical facility, as deemed necessary.

7. Compliance with Rules and Instructions: I agree to comply with all rules, regulations, and instructions provided by the instructor and staff of Strive during the group fitness classes.

I have read this liability waiver and fully understand its terms. I acknowledge that I am voluntarily giving up substantial legal rights, including the right to sue. I understand that this waiver is intended to be as broad and inclusive as permitted by the laws of the state/province in which the group fitness classes take place, and that if any portion of this waiver is held invalid, the remainder will continue in full force and effect. If any portion of this waiver is held invalid, the remainder will continue in full force and effect.
